Support Coordination
Support coordination helps you to understand, implement and optimise the funded supports in your NDIS plan. At Reach focus on linking, you to community, mainstream and other government services to support your individual needs. Our dedicated team will support you to have trusted conversations, connecting you with a range of service providers enabling you to direct your own life and make your own choices.
VOOHC
Voluntary Out-Of-Home Care (VOOHC) is a home away from home. If support is needed to assist with care for children and young people with disability outside the family home, At Reach can assist. VOOHC may be provided as a one-off emergency placement or may be accessed on a regular or long term basis
These arrangements can be funded in several ways including direct payment by families, the bail assistance program or individual funding through the NDIS.
Household Tasks
At Reach will with our participants, supporting them in the maintenance of their home and garden. Whether that means aiding with cleaning or offering some green fingered hands, we have the support available to aid you in leading your best life.
We provide support in activities of daily living. We will support our participants as they maintain their daily routine, help them with house cleaning and house chores, personal care, meal preparation and assist them with cooking and feeding.
SIL (Supported Independent Living)
Supported Independent Living with At Reach means compassionate support from highly trained staff and management who care about you, in an accessible, welcoming home environment. Our participants feel safe and nurtured and benefit from person-centred support that is tailored to their individual needs to help them achieve their goals and live their best life.
As an SIL provider, At Reach offers disability accommodation and daily life assistance so that you can achieve as much independence as possible. We provide 24/7 supported living homes throughout the Sydney region.
Daily Living Assistance
This service helps our participants with basic and essential tasks. From washing and dressing to eating and walking, our expert care providers provide the support necessary to help our participants care for themselves. We also provide arts and crafts, life skills development such as making a cup of tea or coffee, knowledge on how to use transport and how to maintain personal hygiene.
Social & Innovative Community Participation
It’s important to feel connected to something you enjoy doing in or out of your community. That is why we take time to identify participant’s interests and find ways in which to connect them with people, including family, friends and carers, and your wider community. Allow us to help you or your loved one connect with others and find activities, events, and resources which we believe will be key to living a better life.
Occasions where we help our clients to participate in the community include attending doctors, various allied health professionals, and all other appointments, as well as more leisure-based activities such as shopping and socialising with family and friends. We will also enable them to participate in local and group activities such as recreational visits to park, Beach etc, participating in bowling, golf, swimming, and gym and going out for games, events, movies, and concerts.
