We offer a wide range of Out-Of-Home Care Services
Supervised Contact
At Reach is committed to providing positive supervised connection services that allow young people and their parents the opportunity to build & maintain a positive relationship. Whilst we recognise the significant value of the child-parent (significant other) relationship, our primary focus is the safety and well-being of the child/children.
We understand that various parenting situations often struggle to come to an agreement with contact arrangements. At Reach can assist supporting and guiding parents through this process, alleviating stress by navigating to a positive outcome through negotiation in accordance with any current orders or parent agreements. At Reach is a non-biased, neutral third party and we believe that every young person has the right to see both parents whenever possible, in a conflict free zone.
At Reach facilitates both court ordered, and parent agreement supervised contact. Contact is always conducted in a safe, child friendly and appropriate environment in accordance with court orders or by parent agreement. We provide a trained, neutral supervisor to monitor the contact, whether at the park, a movie, or bowling night. The supervisor will discreetly observe and monitor the contact visit and can, on your request, provide a comprehensive report. Our goal is to make every connection as positive as we can for all parties.
Mentoring
Our Mentoring service is tailored to each young person based on their needs and interests and delivered by skilled mentors who have experience and training in working with at-risk youth and vulnerable individuals.
At Reach connects young people with local supports (informal and formal) and provides meaningful ways for them to learn, respond and engage with At Reach and the wider community.
By accessing our service, participants can experience movement towards independence and wellbeing as we work together to develop essential life and relationship building skills and engagement within the community.
Aftercare
Our aftercare program exists to help young people (15-25 years). Leaving care can be exciting and difficult but that shouldn’t mean you’re left without support. Reaching adulthood can be an inspiring time for any young person ready to transition from out-of-home care.
Our Aftercare services have been developed to help you take your next confident step towards independent living. Part of that journey towards independence means gaining the necessary social, emotional and practical skills to enjoy a self-sufficient life.
Working in conjunction with your leaving care plan, we assist by enhancing what may or may not be in place to assist those next steps.
Having someone walk with you after leaving OOHC can mean the world for some but not all. At Reach wants to support you to reach those goals of independence, build your capacities and see you living confidently independent.
Transport
At Reach provides transports services that see each young person/s driven safely from the point of pick up till the point of drop off. Whether it be a school drop off, an appointment, more than 1 young person or driven to camp/event, we’ll get you there and back if.
Before any arrangements are made, we ensure that any specific preferences are considered prior to service delivery. It’s important we collect all information and act accordingly to any special requirements.
VOOHC
Voluntary Out-Of-Home Care (VOOHC) is a home away from home. If support is needed to assist with care for children and young people with disability outside the family home, At Reach can assist. VOOHC may be provided as a one-off emergency placement or may be accessed on a regular or long term basis
These arrangements can be funded in several ways including direct payment by families, the bail assistance program or individual funding through the NDIS.
